If this is somehow the last achievement you need, give yourself a pat on the back!
To quickly rack up deaths, pay a visit to Chapter 11 - Deadline. When you've cleared out all the enemies and are moments away from getting into your car, a van will crash into the building to attempt an ambush. Dying and hitting the retry button will give you a few precious seconds to get out of the way, but if you rush forwards and throw yourself at the vehicle, you can score hundreds of deaths in a matter of minutes.
