That's It? + This Is It
Thankfully, there's no boss fight to worry about in the epilogue - but if you want to see the secret ending, a collectible hunt awaits.

You'll get the first achievement for completing Chapter 19 - Resolution, which sees our new protagonist finding out who, exactly, is behind the phone calls - but not much else. The secret ending is unlocked when you find all of the puzzle pieces - a full guide for that can be found here - and interact with the computer before descending into the sewers.
If you already solved the puzzle, you can still get the regular ending by entering the sewers without interacting with the computer.
